This is a slim volume written by a psychiatry House Officer for medical students. Author Neel Burton's aim was to write a readable book which is "clear and concise yet comprehensive and detailed". The title is a little imposing, but Burton has succeeded in writing an accessible guide for undergraduates. The book follows a standard format of a brief history, with general chapters on patient assessment and mental health services, followed by thirteen individual chapters grouped under diagnostic headings. This will suit medical students who need to learn diagnostic language and standard treatments. The book is attractively presented, with attention to layout and design, and Burton has illustrated each chapter with anecdotes, poetry, quotes from famous people with mental illness, and well chosen classical paintings from Hogarth, Picasso, Dali and others. The chapters are brief, informative, and well organized, there are none of the long, complex pages of text found in the standard psychiatry text books. There is even a list of true/false questions at the end of each chapter, and while specialists might feel that in a few cases the answers could be contested, in the main they are straightforward and factual. A medical student could read this the night before his first psychiatry run and front up to his consultant reasonably well informed.
Books such as this run the risk of oversimplification, but in general Burton has done a good job of providing brief, clear descriptions, well referenced to both ICD and DSM diagnostic criteria. While inevitably medical in its focus, each chapter presents a range of interventions for each disorder, and covers contributing background factors such as family environment and stress, even if they are accorded a secondary role compared to the book's underpinning biological theory. Burton's penchant for formal language occasionally gets in the way of his goal of clarity. Very few people call sleep disorders "dysmonias", and the average medical student is unlikely to hear "anankastic personality disorder" discussed in ward rounds. And there are few examples of accepted psychiatric "truths" presented, without qualification, as factual, such as the "rule of thirds" in relation to schizophrenia.
If the book succeeds in providing a synopsis of the major diagnostic categories, there are a few sections that are too brief. For example the two pages devoted to "Ethics and the law" presents an interesting and challenging case scenario, and Burton does a reasonable job in discussing the issue of capacity and involuntary treatment. But the important issue of ethics in clinical practice deserves more space. Burton does include the rider that the scope of the book precludes a full discussion. The problem is that the only example provided ends in a decision, ethically justified on the basis of the details provided, to opt for involuntary treatment. There is a lost opportunity to demonstrate to students that there are times when psychiatrists need to respect decisions to refuse care.
Medical students will find this book a helpful guide to their undergraduate experience. The book will need to be augmented with wider reading on therapeutic relationships, alternative models of mental distress, psychological interventions and the like. Students will inevitably find that a concise understanding of psychiatry as a clinical science imposes more conceptual clarity than will be encountered in clinical practice. But Neel Burton's Psychiatry succeeds in providing a readable and accessible introduction.
